Online Registration Instructions

The procedure for submitting your Credit Repair Services registration online is as follows:
  1. Download the Credit Repair Services Registration Form
  2. Complete the fillable PDF form and save the completed copy
  3. Upload the completed Form and the requested supporting documentation in the fields provided below. (See the registration form for requested supporting documentation.)
  4. Pay the required registration fee(s) by check to the address listed below. Checks should be made payable to the “Louisiana Department of Justice.”
